Registration

Students must register (or re-register) every year in the Summer before the Fall Term. Before you register, please read the appropriate letter (below), the CCSA Policies and Procedures, and the Group Lesson Schedule and Fees.

To register you must print and complete the following two forms:

Registration Form for 2010-11 as .pdf or
Registration Form for 2010-11 as .doc

CCSA Policy Agreement Form for 2010-11 as .pdf or
CCSA Policy Agreement Form for 2010-11 as .doc

Fill out the Registration Form. A parent (or guardian or adult student) must initial each item on the Policy Agreement Form AND sign and date the form.

We also require a $75.00 deposit. This deposit will be applied to the fall tuition and is refundable until the first week of classes (September 7th).

To register, print the forms, fill them out, and send them with your check for $75.00 to:

Registration materials are due no later than August 27, 2010 for Fall Term 2010.
